Subacromial Impingement Cuff Tendinitis And Bursitis
What is it? There are three terms here that are often talked about separately but are really part of the same problem: Cuff Tendinitis, Bursitis, and Subacromial Impingement. Subacromial Impingement is when the rotator cuff tendon gets squished under the acromion, leading to cuff tendonitis and inflaming the bursa. Its a downward cycle of inflammation leading to poor function, causing more inflammation, and so on.
What are the symptoms? Usually a sharp pain in the shoulder when you put your arm overhead. Sometimes its worse when you are bringing the arm down. It can ache at night, especially toward the side of the shoulder. There may be some crunching. And you have sharp pain with exercise.
How is it treated? I personally treat this problem by trying to break both sides of the downward cycle. A shot of anti-inflammatories or steroids will reduce the pain and inflammation. And physical therapy will restore good function. Oh and also, stop abusing the shoulder. I once had a patient doing 300 push-ups a day, every day! There is nothing I can do to fix your shoulder if you dont stop overworking it!
Other Causes Of Pain When Lifting Arm
There are a few other more unusual conditions that can cause shoulder pain when lifting arms
- Brachial Neuritis: inflammation of the nerves at the top of the shoulder in the brachial plexus
- Labrum Tears: damage to the ring of cartilage that surrounds the shoulder socket. Upper labrum damage is called a SLAP tear and lower labrum damage is known as a Bankart Lesion

Osteoarthritis Prolonged side sleeping may cause cartilage breakdown in the shoulder or inside the acromioclavicular joint of the shoulder. Pain Around The Upper Arm And Shoulder
This happens when the pain is localized predominantly in the area of your shoulder and upper part of your humerus. The conditions that cause this can include:
- Rotator cuff disease: Initially, pain is often felt at the front of the shoulder.
- Torn rotator cuff: Tears in the rotator cuff can include pain that begins near the shoulder.
- Dislocated shoulder: Pain from a shoulder dislocation is most acutely felt near the shoulder.
- Broken collarbone: The pain from a broken collarbone can be felt close to the shoulder, particularly when you try to move your arm.
- Frozen shoulder: This type of pain typically comes on gradually and feels dull and deep.
- Calcific tendinitis: Pain due to calcific tendinitis can be severe and can come on suddenly, often in the morning.
- Shoulder sprain: You can feel pain from a shoulder sprain not only in your shoulder, but also in the area of your upper arm and collarbone.
- The pain due to a separated shoulder is felt around the shoulder and upper arm.
- Shoulder osteoarthritis and RA: People with arthritis in their shoulder often feel a deep ache in their shoulder and upper arm.
- Brachial neuritis: Pain from brachial neuritis is typically severe and most often happens in the area of the shoulder and upper arm.
- Brachial plexus injury: Pain from a brachial plexus injury can happen around the shoulder and upper arm. It may be long lasting.
Why Does My Shoulder Hurt After Sleeping
If you sleep on your side and wake up with sore shoulders, try adjusting your sleeping position. Avoid sleeping on the same side as your pain, which may worsen the problem. Instead, sleep on the other side or on your back to distribute weight evenly and relieve pressure on the shoulders. If you must sleep on your side, roll backwards slightly.
A bad mattress, improper sleeping position, or underlying shoulder condition can cause shoulder pain during sleep. You should choose a medium-firm mattress and a good pillow to minimize shoulder pain. You can also rest your shoulder and take anti-inflammatory medications. If nothing seems to help, talk to your doctor.

What Are The Treatment Options For Shoulder Pain
Treatment will depend on the cause and severity of the shoulder pain. Some treatment options include physical or occupational therapy, a sling or shoulder immobilizer, or surgery.
Your doctor may also prescribe medication such as nonsteroidal anti-inflammatory medications or corticosteroids. Corticosteroids are powerful anti-inflammatory drugs that can be taken by mouth or your doctor can inject into your shoulder.
If youve had shoulder surgery, follow after-care instructions carefully.
Some minor shoulder pain can be treated at home. Icing the shoulder for 15 to 20 minutes three or four times a day for several days can help reduce pain. Use an ice bag or wrap ice in a towel because putting ice directly on your skin can cause frostbite and burn the skin.
Resting the shoulder for several days before returning to normal activity and avoiding any movements that might cause pain can be helpful. Limit overhead work or activities.
Other home treatments include using over-the-counter nonsteroidal anti-inflammatory medications to help reduce pain and inflammation and compressing the area with an elastic bandage to reduce swelling.
Pain Between The Shoulder And Elbow
In some cases, pain can be felt in both the area of the right shoulder and further down towards your right elbow. Some causes are:
- Rotator cuff disease: Pain can begin to radiate from your shoulder into the side of your upper arm.
- Torn rotator cuff: Shoulder pain may radiate down your arm to the area of the elbow.
- Dislocated shoulder: The pain from a dislocated shoulder can radiate down the upper arm.
- Upper arm fracture: A break in your upper arm can cause pain thats localized around the area of your shoulder and throughout your upper arm.
- Frozen shoulder: Pain from a frozen shoulder may also spread into your right bicep.
- Calcific tendinitis: Similarly to frozen shoulder, pain from calcific tendinitis can spread down into your bicep area.

Pinched Nerve In The Neck

It can be caused by a traumatic injury, such as from sports or an automobile accident, especially if the injury results in a herniated disk. It may also arise from the normal wear and tear of aging.
Symptoms include sharp, burning pain with numbness and tingling from the neck to the shoulder, as well as weakness and numbness into the arm and hand.
Diagnosis is made through patient history, physical examination, and simple neurological tests to check the reflexes. Imaging such as x-ray, CT scan, or MRI may be done, as well as electromyography to measure nerve impulses in the muscles.

Path To Improved Health
Your doctor can help you with a treatment plan to relieve the pain and help you restore your shoulder to normal function. Pain relief strategies include active rest. During active rest, you can and should move your shoulder. Avoid difficult activities like lifting heavy objects or repetitive overhead movements. You may also get relief by applying ice, taking nonsteroidal anti-inflammatory medicine such as ibuprofen or naproxen , and, occasionally, an injection of anti-inflammatory steroids. Several issues can cause pain on the outside of the shoulder. The most common cause is a tear in the rotator cuff, which helps lift and rotate your arm.
Rotator Cuff Problem
Four rotator cuff muscles move your shoulder, including the deltoid muscle. There are three main types of rotator cuff problems:
Rotator cuff problems cause pain, especially with activities like reaching or throwing. The deep, aching shoulder pain tends to worsen or flare up at night. It can easily wake you up from sleep or make it difficult to fall asleep.
Rotator cuff problems can be associated with a limited active range of motion. Since it’s hard to voluntarily move your injured or inflamed muscle, your shoulder may feel stiff.
But if someone does the work for you by lifting your arm , your shoulder will likely move normally.
Research suggests that high blood pressure, as well as other heart disease risk factors are associated with rotator cuff pain and inflammation.
