What Is Pancoast Tumor Treatment
The optimal treatment for Pancoast tumors has evolved. All lung cancer treatment continues to consider the patient’s overall condition and what is specifically called performance status in making treatment decisions. The performance status compares the patient’s condition to their pre-illness presumably normal activity levels. In patients with limited tumors and early Pancoast syndrome and good performance status, today the ideal treatment of even potentially operable cancers is a combination of cisplatin-based chemotherapy and radiation.
This is then followed by restaging, and when appropriate, aggressive surgical resection of the entire lung and affected adjacent tissues followed by further postoperative chemotherapy. This approach can be curative in as many as half of patients followed for five years or more.
Alternative approaches in selected patients including surgical resection alone, preoperative radiation and surgery , or just radiation alone can be considered in selected cases. The best results have been reported with combined multi-modality treatment.
What Does Lung Cancer Shoulder Pain Feel Like
Shoulder pain from lung cancer is quite similar to other forms of shoulder pain. Because of this, it might be difficult to determine the cause of your shoulder pain.
If youve recently fallen or injured your shoulder in some way, lung cancer is unlikely to be the cause of your shoulder pain. There are many reasons for shoulder pain that arent lung cancer.
Lung cancer is more likely to be the cause of your pain if youre a smoker and your pain:
- occurs during rest
- isnt associated with any strenuous activity involving the shoulder
- happens at night
- doesnt resolve itself after a few weeks
Your shoulder pain is also more likely to be a symptom of lung cancer if you have other lung cancer symptoms as well. These might include:

Types Of Pain In Lung Cancer
Pain is generally considered the most common symptom of lung cancer. Pain is often caused by multiple factors and its management needs to be multi-disciplinary to address all these aspects. In cancer, pain can be characterized into two areas: acute pain and chronic pain. Acute pain is usually due to a definable injury or illness. Acute pain may be a secondary cause of cancer, such as a severe bleeding into a tumor, bone pain caused by a metastasis , or abdominal pain caused by a metastasis that obstructs the bowel. Acute pain is usually experienced for a limited duration and is predictable. It is usually associated with clinical signs such as increased heart rate, high blood pressure, sweating, and pale skin.3
Chronic cancer pain may be a result of the same causes as acute pain but it is experienced for a much longer duration. Chronic pain can persist for more than 12 hours a day and often has a gradual or poorly defined onset. Approximately 75 percent of cancer patients live with chronic pain.3
Many cancer patients experience flares of pain despite using pain medications. These flares are called breakthrough cancer pain. Breakthrough pain greatly impairs a patients quality of life and can cause additional psychological burden.3
What Is The Life
In the past, Pancoast tumors were considered inoperable and incurable because of their relative inaccessibility and extensive invasion of nearby tissues and structures. However, recent studies have shown that, in some persons, the tumor completely stops growing and the pain is gone. In addition, survival rates have improved. Chemotherapy and radiation can result in no evidence of viable cancer in the surgical specimen.
Clinical studies demonstrate that even preoperative irradiation alone in doses not strong enough to shrink the tumor:
Factors that are associated with a poor prognosis include Horner syndrome, mediastinal lymph node involvement, and incomplete surgical removal of the tumor. To date, no patient with any of the above factors has survived for five years.
Factors that hurt outlook can be summarized as:
- Presence of Horner syndrome
- Involvement of mediastinal lymph nodes
- Involvement of supraclavicular lymph nodes
- Invasion of the tumor into areas of the vertebrae

Signs Of A Pancoast Tumor
Shoulder pain can also be a sign of a Pancoast tumor , a rare type of lung cancer that begins in the rounded upper section of the lung and spreads to nearby areas of the body. Although a Pancoast tumor is a form of lung cancer, individuals with this condition generally dont experience any respiratory issues. Instead, they may notice:
- Pain in the shoulder, arm, armpit, upper back or between the shoulder blades
- Muscle weakness
- Facial flushing
- Excessive facial sweating
In some instances, someone with a Pancoast tumor may also develop the symptoms of Horners syndrome, which include drooping eyelids and reduced pupil size.
Really Worried About Shoulder Pain
I just joined this forum after worrying for weeks with shoulder pain.
To start at beginning I was experiencing coughing up blood, breathlessness and fatigue with pain also weight loss..for quite sometime..eventually my doctor sent me for chest xray..then got sent for ct didnt hear back..called surgery..told I had lesion on lung at top of lung near spine..had referral to lung clinic..had ct with dye..told left lung deflated and mass at bottom of lung?…so thats confusing..doc said cant say if lung cancer or not..so will need another ct in few months to monitor nodule..which is walnut sized..i never smoked…but have asbestos in my home..some of which has been removed whilst i was in house so couldnt leave house..
Now I have had pain in my left shoulder/blade for a few weeks now..also arm pain and weakness…and overall backpain..aches..I am getting worried as it is quite painful and disturbs me at night and is painful without using it..
Getting an appt with gp takes weeks…months and I’m in pain and getting worried the pains related to possible lung cancer…also noticed little lumps on my ribs…
Please can anyone tell me if theyve had similar experiences and what the outcome was…I woyld appreciate any advice info from nurses or anyobe with similar symptoms..

- Chest X-ray film:
- In the early stages, Pancoast tumors are difficult to detect on chest X-ray films because the top of the lung is located in an area of the body that is difficult to visualize clearly on an X-ray film. Shadows that lie over the lungs make the image on the X-ray film unclear. Many patients end up consulting orthopedic surgeons and neurologists before a definitive diagnosis is made. An apical lordotic view ordered with the chest X-ray may visualize the problem in patients with suspicious symptoms.
- A chest X-ray film may reveal asymmetry of the tops of the lungs in the form of a small, uniform patch of tissue or pleural thickening on the apex of one lung. The X-ray film may show a thin plaque at the lung apex in the area of the superior sulcus, or the X-ray film may reveal a large mass, depending on the stage when it is first diagnosed.
- The plain chest X-ray film may show that the tumor has invaded one or more ribs or parts of the vertebrae. Bone destruction of 1-3 of the back ribs may sometimes be visible on the X-ray film.
- Enlargement of the mediastinum may be visible on the X-ray film.
